FID RULES/CONSTRAINTS:

1. T, F, and P order types prohibited for Access Service application.

SERVICE ORDER ACTIVITY NOTES:

1. The listed name entry may be used in lieu of a Billing Name in the Billing Section if CRIS can follow predefined rules to edit a listed name to a billing name, or if the listed name differs from the billing name only because:
   
     - listing instruction codes appear in parentheses,
   e.g., (NON-PUB), (NON-LIST), (DNA), etc.
   
     - the listing contains an asterisk for capitalization purposes
   
     - the listing contains a designation (unless the customer requests
   requests that the designation become part of the bill name).
2. Billing name and address information, including post office information, is limited to a maximum of 5 lines.
