SERVICE ORDER ACTIVITY NOTES:

1. A casual toll user is defined as a customer who accesses an Interexchange Carrier's facilities, other than the presubscribed carrier, by dialing 10xxx.
2. Numeric designation (Carrier Identification Code (CIC)) may apply for the instance of an IC with multiple access numbers all associated with the same access customer name abbreviation.
