FID RULES/CONSTRAINTS:

1. T, F, and X order types prohibited for Access Service application.
2. X action code prohibited on Access Service orders.
3. This FID is for billing purposes only.  It is not used for provisioning.
4. Monetary amounts are always displayed in dollars and cents without dollar signs or decimals. For example: 075 = seventy-five cents; 100 = one dollar; and 10000 = one hundred dollars.

SERVICE ORDER ACTIVITY NOTES:

1. The telephone number (elements 5 through 7) is the number to which one-time debits or credits are to be applied when the number differs from the normal billing TN.
