FID RULES/CONSTRAINTS:
  1. Section:  ASGM    Position:  FL
    This FID is entered in the Miscellaneous Subset, Central Office Facilities Subset, and Outside Plant Facilities Subset.
  2. Section:  STAT    Position:  LH
    Each DPA in the S&E Section must be entered separately in the Statistics Section with the statistics for that DPA entered following the left-hand FID Central Office Completion (COC) or Order Completed By (OCB) entered beneath the DPA FID.
  3. Section:  STAT    Position:  LH
    When statistics are reported for both the LA/SA and a DPA, the statistics for the LA/SA must be entered in the Statistics Section before statistics for any DPA.
  4. Each address is assigned a unique numeric identifier for the account.  Consecutive numbering is not required.
  5. On C, R and X order types, the DPA FID may be shown recapped without an action code.
  6. T, F, and X order types prohibited for Access Service application.

SERVICE ORDER ACTIVITY NOTES:
  1. The SLA FID is used when an address for a Primary Termination of Service is in addition to the customer's main address.
