SERVICE ORDER ACTIVITY NOTES:

1. The LTPV FID is entered on a calling plan USOC line to denote that a change in IntraLATA Carrier is awaiting third party verification.  The Predesignaged IntraLATA Carrier (LPIC) FID, which will ultimately express the verified carrier choice, does not appear on this service order.
2. After third party verification has occurred, a second service order containing the verified LPIC FID is used on the assignable line USOC to provision the IntraLATA carrier choice.
