   For Access Service Tariff application, either Elements 1, 2, 3, and 4 or Elements 1, 2, 3, and 5 are required for handling individual case filings with other than a one month minimum period.
2. T, F, and X order types are prohibited for Access Service application.
3. X action codes are prohibited on Access Service orders.
